Question: for several months, I awake with dull pain inside rib cage, on left side, under shoulder blade. I sleep on my back or right side, changing positions 3-4 times per night. I am 79, and have multiple back surgeries but this pain in not in surgical area

Noted the history and understood the concerns.
Dull pain is inside the rib cage - on left side - under shoulder blade - multiple back surgeries but pain not in surgical area...
All this indicate that there may be a problem with the lung and/or pleura on the affected side.
I would advise you the following in such a situation:
Clinical evaluation and examination by a Doctor particularly to hear the breathing sounds called auscultation. It can show peculiar sounds and indicate for any internal problem.
X-ray of the chest in PA and left lateral position as a preliminary investigations to be followed by CT scan if anything is required to get an appropriate diagnosis so that an appropriate management can be done.
