Question: Hello Doctor,
My 9 month old kid got some reddishness under his hips. this may be due to excessive diaper use. can you pls assist for the same. i have also attached images for the same.

Hi
Thanks for writing to us.
I have seen the attached picture.
Based on your description and image it could be excoriation from stools and secondary bacterial infection of damaged skin.
Let me know his history
1. Did he have loose stools recently?
2. Is he on cows milk?
3. Does he get this frequently?

Though diapers can aggravate this perianal rash it is more likely caused by lactose malabsorption after an episode of diarrhoea. The undigested lactose is responsible for skin excoriation. Once damaged the skin will get infected by bacteria.
I would suggest
1. Avoid cows milk or lactose containing formula.
2. Use lactose free milk like isomil.
3. Restrict diaper use only for few hours in the night alone.
4. Use zinc drops at 10 mg per day which I usually prescribe for 2 weeks.
5. I also advice such patients Antibacterial ointment like fusidic acid along with emollient like happynap daily till it heals.
Avoid cows milk till one year.

thanks doctor. You are right we are giving him cow milk.I f stop cow milk can we give him XXXXXXX milk or packet tonned milk

No need for cow or XXXXXXX or toned milk.
All are not recommended. Best thing is to try foods when you are not at home and breastfeeding once you are at home.
If child is adamant about milk, you can give packet milk ( as last resort)

Double toned will be poor in nutrition.
