Suggest Treatment For Diarrhea And Gas In Infant

Question: My baby boy is 6months old. He is having diarrhea (Green watery/greasy stool 10 times a day, Gas, His mother does not get enough milk, so doctor recommended Dexolac1, but he does not gain weight, at the age of 4months we are giving him Cow milk and that suits good to him.

Earlier he has diarrhea 14 times in 24hours, now he has 7-8 times a day, but 7 days are passed. How to improve. He loss 160gms weight in 8 days.

The common cause of diarrhea in this age is viral infection or intolerance to any food item.
At 6 months of age the total requirement of fluid and calorie exceeds the supply by mother's milk.
That is why complementary feed should be started at this age.
During diarrhea there is loss of inner layer of intestine which helps in absorption of lactose and this causes transient lactose intolerance.
It is better to avoid lactose containing product as this aggravated the loose stool.Both Dexolac and cow milk contains lactose.Cow milk in addition contains some protein which can cause cow milk protein allergy and thus should be avoided below 1 year of age.
From your medication history I got to know that you are already giving enterogermina(probiotic which helps in maintaining the gut flora lost during diarrhea),emeset(for vomiting)ORS(to maintain the hydration)curd(rich in probiotic).I would suggest you to continue all these medication and to give zinc syrup in addition.Zinc helps in early recovery from loose stool by normalisation of inner layer of intestine.
I would like to give soya based formula(like Isomil) for few days to avoid any lactose malabsorption.
It is usual to loss weight during diarrhea and this is mainly due to fluid loss from the body.So keep him hydrated well by giving feeds and ORS. Please ensure that he should pass at least 4-6 times urine per day.He will recover his pre illness weight within 4-5 days if hydrated well.
As he has completed 6 months, I suggest you to start semisolid food like rice water,mashed potato,banana,mashed boiled apple. Introduce cereals one by one(wheat in last) at interval of 4-5 days starting from very small amount.

My boy do very loose poo, approx. 8 times a day, and after each feed or eat he does a poo except mother's milk.

He has red rashes on buttocks and around anus area. Whenever he passes stool or pee, he cries a lot.

Which zinc syrup to be use. If you can advice.

Passing stool soon after eating something is gastrocolic reflex which gets aggravated in case of lactose intolerance(also known as rectal urgency).The main aim is to prevent dehydration by ensuring good amount of oral fluid intake.
Once there is loss of absorptive layer of intestine it takes usually 7-10 days(sometimes 14 days) for regeneration of that layer and thus the stool pattern to get normal.Zinc helps in early regeneration.The dose is 20 mg/day(5 ml contains 20 mg).You can get it from the chemist shop with brand name of Zincris.
Red rashes around the anus again is suggestive of lactose intolerance(excessive gas also is a sign of lactose intolerance) due to irritation of skin with unabsorbed lactogen.
Application of ointment like Rashfree or EziNapi and keeping the area dry will relieve pain and also help in healing the rash.Don't rub but clean the area with gentle tap(with the help of baby napkin).Avoid lactose containing formula.

Is this lactose intolerance is temporary or may be due to amul milk. However I am giving him amul milk from last two months and in last two months he has got 2.3kg weight after being on Amul Cow Milk or Diluted Amul Gold Milk.

I tried lactogen 2, but my boy cries a lot after consuming lactogen 2.

What food should I give till diarrhea persists?

Can I continue giving Amul Milk to my boy after diarrhea is cured?

This is transient lactose intolerance

You are right.This lactose intolerance is temporary that is the reason it is known as Transient or Secondary(Secondary to diarrhea) lactose intolerance.
Lactose containing product should be avoided for few days only and then you can continue the milk(If tolerating cow milk well).

You can give rice water,dal ka pani,cereal prridge,curd,mashed banana and mashed boiled apple/potato.All this are tolerated very well in diarrhea.
