Suggest Remedies For Soft Spot In An Infant

Question: Hello doctor, my baby girl is 10 month old ...my concern in her soft spot...still it is open...its about 2 inches open what i feel...is it normal by this age to have this much open soft spot? She is a preemie baby born in 35 weeks 3 days...should i be concerned or it will fill by the time...i have one more question she is sitting on w position what i read on net is cqlled as w sitting...so is this also a cause of concern...my father in law tells that both her father and twin uncle used to sit like this...is it some cause of concern.? If it is then please suggest some exercise so that i can fix it up as early as possible...i am worried about this

I have gone through the details and do understand your concern.

Sitting in W position is quite normal in children.Children usually sit in the position which they find comfortable for them.
It's quite easy to achieve balance in W posture and that is the reason during development child may prefer this position.The thing you should take care of is that this posture should not be consistent and she must sit in other postures too like crossing the midline,both the legs spread out etc.
I won't get overly worried if she is comfortable in other positions too and there is no restriction of movement (at knee or hip joint).
Now coming to the persistence of enlarged soft spot;as you have mentioned the size of 2 inches it is quite unusual.Although prematurity is an important cause of persistent wide fontanelle,2 inches (5 cm) is unusual.The soft spot or anterior fontanelle gets closed by 9-18 months.It varies from baby to baby and the average time for its closure is around 14 months.
As the size of fontanelle is large the other treatable causes like vitamin d deficiency and hypothyroidism should be ruled out.If the reports are normal I would suggest you to observe the baby and wait for the closure.

Sorry doctor the fontenelle is 3 cm as my husband measured it...is it too.large..should i talk to her pead? I am giving her vitd mashyne drops right from her birth...she is all active and reaching her milestones almost on time as per her adjusted age...please advice

Fontanelle of 3 cm size is normal at this age for a premature baby.Continue Mashyne drops and get her evaluated for thyroid hormone status if not done earlier.
Don't worry,monitor for her developmental status and also observe for closure of Fontanelle.

Sir what should be the dose of mashyne per day at her age...i am giving 5 drops per day...it is mashyne 3000iu...also i am giving ger vitcofol drops...

Sorry for delayed reply

The daily requirement of vitamin d for a premature baby is around 800 unit.Each ml contains around 20 drops.So 5 drops of mashyne which contains 750 unit would be appropriate for her.
Iron should be supplemented along with other vitamins in premature baby till one year of age.Vitocofol is multivitamin and multi mineral drop.So continue the same.
