Question: I had a degloving surgery for a fractured penis and torn urethra. Everything is working and it has been 11 weeks. However, my penis does not feel the same, there is a lot of hard scar tissue under the skin, and the scar and swelling under the head are still very noticeable.

1. Have you performed this procedure before?
2. Are these common side effects?
3. I have a thrombosed vein, how can I get rid of this? Is it possible it will be there forever?
4. Will the scar tissue get better, worse, or stay the same?
5. How long should I expect to wait until my penis is as recovered healed as it will ever be?

Thanks for your query ,based on the facts that you have posted it appears that you have undergone surgery for repair of fracture penis 11 weeks back and now have some issues in penis .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you get normal hard erection after surgery ?
2) Was surgery done immediately (within few hours)after you sustained fracture of the penis ?
3) Is the thrombosed vein causing pain after erection ?

I would answer your queries point to point as detailed below.

1) I have performed 7 degloving surgeries for fracture of penis in my 43 years of practice .

2)The tenderness and swelling at the site of incision and scar tissue is a part and parcel of healing process which gets resolved gradually within 6 months .

3) Thrombosis of lateral superficial vein of penis is not uncommon and can occur even in normal individual and should not be worried for if it does not pose difficulties in performing sex or is painful .You can use topical ointment like Thrombophob ointment for local application which helps to get thrombus dissolved.

4) Scar tissue is bound to get softened gradually and you will not notice it .You may feel a small nodule under the skin where the rent in Tunica Albugenia has been sutured .This is due to fibrosis around nonabsorbable suture material being used to suture the rent .

5) You can expect 6 months for complete recovery and have normal enjoyable sexual life .

Thank you for your response. Below are the answers to your questions.

1) Do you get normal hard erection after surgery ? Yes, they might not be as hard as they once were. Do you think this will get better with time?

2) Was surgery done immediately (within few hours)after you sustained fracture of the penis ? The surgery was done approx 7 hours after the fracture.

3) Is the thrombosed vein causing pain after erection ?
No, the vein is not causing pain. It is just hard and noticeable when erect. It is embarrassing...

Is there anything I can do to minimize the visibility of the scar from the surgery? It is pretty red and has some raised parts.

2. Am I more sucseptible to another injury because it has been repaired?

3. I've experienced no urethral stricture after 11 weeks. Should I keep an eye out for that in the future?

Thanks for follow up .

1)In majority of cases what you think it to be thrombosed vein is not thrombosed vein but in reality it is dilated Lateral Superficial Vein of penis which becomes more prominent on erection of penis .There is nothing to be worried about it .

2) There are no chances of injury again unless you have rough and vigorous sexual encounter in woman on top position .

3) It does not lead to stricture formation unless the urethra is also injured with fracture of penis (Which happens very rarely)

In view of having urethral tear possibility of developing stricture of the urethra can not be ruled out .However it depends mainly on the length ,site and extent of the tear .

Since urethral injury has been repaired within reasonably short time after injury you need not worry .Keep a watch on your urine stream and consult your Urologist moment it becomes weak and thin or you need to strain for urination so that the issue can be sorted out by simple procedures like Visual Internal Urethrotomy (VIU).
