Suggest Remedy For Abdominal And Back Pain When Under Stress

Question: Hi. Ive been undergoing lots lf stress lately and as ive written in ive had lower right side abdominal pain, cramp that flutauates between my lower and upper abdomen. As of the last few days the pain is more of a cramp like sensation or feeling like ive been kicked in the stomach. I notice this when i am stressed and then i also have the back pain. My doctor says its muscle skeletal and a symptom of anxiety. 7 years ago had cat scan, colonoscopy as i had stomach pain as well all normal. This seems worse but doctors dont want to repeat all these tests. Only had ultra sound lower abdomen which was normal, rectal exam normal and complete blood work all normal. Doctor says cramping, gas pains is muscle skeletal and anxiety. Is this possible? Thanks

I read your query and understood your concerns.

Your symptoms seems to be related and are typical for IBS (irritable bowel syndrome). Stress cause an increased bowel movement causing a shortening of intestinal transit which is manifested
with spastic colitis pain, change in bowel movement and stools consistency (frequent change between constipation and diarrhea).

Can you please tell me if you have frequent change in bowel movements?

If you were my patient I would like to recommend the following measures:
- avoid alcohol, caffeine and energy drink
- avoid spicy food
- use simeticone to decrease gas forming
- use chamomile tea
- use antispastic ( Buscopan )
- use anxiolytic ( xanax )

I usually go to the bathroom (bowl movement) once a day, however sometimes twice which is different from past years where i was always constipated. Ive never had the cramping like i have now in my stomach so it feels strange to me. I do feel it more when im anxious or worrying about it. Also, ive had some vaginal discomfort lately and burninv when i pee and discharge which doctor said is likely a yeast infection. That goes away once its treated and they said if it were anything more serious the ultra sound would have showed it. Tested urine and jts ok

Hi back,

After reviewing the new data I would like to say that apart from above suggestions(diet and life style change) you should do upper endoscopy. This is because you are experiencing stomach pain and you don't mention to have done it.

Also your symptoms in rare cases may be related to urogenital tract infection, but since the echo and urinalysis didn't find anything wrong than infection has a low probability to cause your symptoms.

Hi! They got the results of the urine test quickly and said its fine so that would mean i do not have a UTI correct? I was wondering bc normally i thought it would have to be sent to a lab but they got the results within minutes. Is this possible? I had an upper endoscopy 7 years ago as well and it was normal so i agree with you - it looks like IBS. So i should just treat the yeast infection i would assume and if urine culture was normal i dont have a UTI correct?

The fact that your quick urine test was normal reduce to minimum the chance to have an UTI.

However a normal urinalysis (that go to laboratory) and an urine culture are essential to exclude any UTI. You shouldn't start antibiotics without test results. If the test will show sign of UTI than you should start any antibiotics.

It's a great thing that your endoscopy was normal 7 years ago, but you should repeat the procedure in order to confirm the diagnosis of IBS.

Hi dr shehu! So i went for another urine analysis amd they noticed 75 white blood cells. Doctor said thAts not very high but gave me amoxcillin to start tomorrow. Is this a veey high count? Now im woreied about a kidney infection bc of back pain but my doctor said dont worry just take the antibiotic. Do you agree?

You are reporting that urine anlysis shows an increased number (75/mm3) of white blood cells. This is a certain sign for urinary infection.

However I would like to check by myself all your medical data. Can you upload those data for me so I can checked them by self and give you an accurate opinion?

For the moment I would like to suggest following advice:
- use plenty of water
- keep a good hygene of vaginal region
- use cammomile tea to wash urinary and vaginal region
- start to use combination of Bactrime (sulfamethoxazole and trimethoprim) and amoxicilin.
- repeat urine anlysis after you have finished antibiotic treatment.
