Suggest Remedy For Blackout Episodes And Arrythmia

Question: my daughter is being tested for arrythmia, there have been two blackouts in 10 weeks with a minor head injury received in each fall. The faint is atypical in that it is like a light switch off and then on again. First she was given a complete neuro work up now it is focused on the coronary. She has no diagnosis but has had her DL pulled. She wont be given a state disability finding without a diagnosis, according to the cardiologist. the real is that without the disability finding she will continue to drive so she can feed her family. I'm sorry if this is not the type of question intended to be asked. Thank you for your time. XXXX

Detailed Answer:
Hello XXXX!

Thank you for asking on HCM!

Regarding your concern I would like to explain that, when dealing with syncope causes, a differential diagnosis by exclusion should be done.

After excluding possible neurological reasons (some types of epilepsy, embolic episodes, etc), cranial blood supply disorders (arterial stenosis, subclavian steal syndrome, etc), eventual structural heart disorders should be sought.

Heart rhythm and conductance disorders may be a potential reason. As the episodic blackouts have been quite apart from each other, a prolonged events recording seems to be appropriate.

Possible neurovegetative disorders (orthostatic intolerance) may be responsible.

A Head up tilt test would be helpful to clarify this issue.

Other causes like hypoglicemia, etc. should be investigated too.

Finding the right etiological cause, and consequently the diagnosis, would help to define the state disability too.

Hope to have been helpful to you!

Feel free to ask me whenever you need! Greetings! Dr. Iliri
