Suggest Remedy For Chest Palpitations While On Nexito And Restil
I had intense chest palpitations last week and was given ciplaLA 40mg and with in one hour , they got subsided. ECG came normal and was suggested to stop taking the drug from next day onwards and docotr gave me an event recoder. Today ECG of event recorder also came normal. So doctor suggested to go with Restil 0.25mg in SOS and nexito 10mg at night. But From morning I am suffering from intense palpitations and Restil 0.25mg did not work. So, after 2 hrs of taking restil , i took ciplaLA 20mg but still suffering from palapitations. So please suggest what should I do know? Shall I take one more 20 mg of Cipla LA or take Nexito to night?

Irregular heartbeats need to be DOCUMENTED by ElectroCardioGram (EKG). Standard ECG records the electrical activity of the heart, for a few seconds. It can detect the arrhythmia occurring during that time only. If the episode occurs randomly one needs Holter or event monitor. Holter records for days; Event Monitor for weeks… Holter records continuously; Event Monitor only at certain times
Here, the recording has to be extended for a longer time….
Ciplar LA 20 is Propranolol.. The dose is 20 to 320 mg! – it has to be tailored to the individual. DO NOT INCREASE THE DOSE BY YOURSELF… It can slow down the rate and bring down the pressure…
The treating doctor will adjust ….
Nexito is Escitalopram and again – should not be increased without doctor’s examination
Restyl is Alprazolam may help - you may try another 0.25 mg and then go to your doctor.
Valsalva manoeuvre may help / your doctor will explain and demonstrate… it is like breath holding and straining (as in the toilet!) / It should be done under doctor’s observation for the first time….
So long for rescue
There are other measures for prevention of recurrence / or aborting it – based on the results of further investigations….

Yesterday, I took one more Restil 0.25mg and went to Emergency while having palpitations and took ECG. As told by doctor, ECG shows Synus Tachycardia with extra beats. I have attached ECG image for your Quick reference. You can observe at the centre of image between 2 QRS intervals, the duration of graph is less. Heart rate during palpitations was 130 / 90.
Finally, my doctor said its purely anxiety realted issue and asked me to start nexito 10 mg at night and continue for several months and in case of SOS go with Restil 0.25mg.
But my question to you is. What do you recommend for treating anxiety related palpitations ? Either a Beta Blocker like Propranol or SSRI like Ecstialopram ? My personal experience is that, I feel better on Propranol 40mg, with no other side affects. But Alprazolam and Ecstalopram are troubling me with side affects such as tingling feeling in hands and feet, weak XXXXXXX through out the day and mood fluctuations.
Which of these medications would benefit me in long run ? I mean i am ready to take Nexito, and bear side affects if you say that it would help me in long run. My age is 31 now and I am planning to get married in another 3 months time.
So please suggest

Ectopic beats can arise from ventricle (lower chamber of the heart - Ventricular Ectopic; VPC) or Atrium (upper chamber - Atrial Ectopic; APC). – as is here in this ECG. There is just one single premature beat. A beat comes earlier than normal – this itself is not usually felt. The next beat is a bit delayed - longer time to recover from early extra beat – so heart fills more and the beat appears forcible – thud / lubb - - dubb - - lubb - - dubb - lub - - - DUBB Rate above 100 is technically labelled tachycardia and here the rate is 96
Many cases of palpitation are benign, especially when associated with normal ECHO and may not require treatment, If you also have symptoms like blurred vision, dizziness, loss of consciousness, shortness of breath, chest pain along with this, you need urgent work up. You do not have these and rest assured, it is harmless….
Sinus tachycardia is a common and the most common cause for it is anxiety
The treatment is appropriate
For further management of Anxiety, the best option is Psychiatric assessment and assistance.., Apart from drugs, psychological counselling could also help. Beta-blockers are good for both anxiety and palpitation; and the first line of management. Next in line is the citalopram… tingling, mood and weakness – may all be from anxiety, and may not be from the drug; also it is too early to ascribe to the drug – it takes a fortnight or more for the drug to have effective results…
As it is, there is no concern about marriage .., you can have a long and happy life / learn to ignore the incidence and enjoy life …
