Older people can have a severe infection that presents only with confusion and not the typical symptoms of an infection. Problems with oxygenation such as a lung problem or blood disorder can be causing it. A stroke is still possible.

Metabolic problems are also possible. These could result from extremely high blood sugar, liver problems, abnormalities in electrolytes, and many other things. A recent heart attack is also possible.
