Suggest Remedy For Constipation In A Hypertension And Diabetic Person

Question: i have problem of acute chronicity of constipation ,stool is not formed properly and lack of intestinal tonicity. I am a diabetic with mild hypertension taking metformin+glimeride ,losartan+hydrochlorthiazide

Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

1. You are possibly suffering from intestinal enteropathy secondary to neuronal dysfunction in the large bowel along with impairment of gastrocolic reflex. This has resulted in constipation.

2. You need to get a thyroid hormone profile, if it constipation becomes chronic to rule out hypothyroidism.

If you were my patient, I would suggest:

1. Good hydration
2. Regular physical activity
3. Increased fiber intake - particularly insoluble fibers from whole grains, vegetables and fruits - atleast a minimum of 30g/day.
4. Additionally, occasionally as necessary sorbitol or lactulose can be used for severe constipation (particularly if >2days).

The laxatives needs to be taken under guidance of a doctor especially saline or osmotic laxatives.
Keeping well hydrated will make the stools softer.

Recently pyridostigmine 60 mg three times daily, regularly is known to help with constipation with diabetes. You need to discuss with your doctor regarding this, before you start.
