Thanks for your query ,based on the facts that you have posted it appears that you have sustained injury to your scrotum during motorcycle accident 3 years back and since then you have noticed your scrotal sac has become black.

1) Was there swelling of the scrotum immediately after accident ?
Black colour pigmentation of the scrotum and penis is mostly due to sub epidermal
bleeding which has stabilized and developed into black patch over scrotum and penile skin .
Since it is long standing and you do not have any symptoms it should be left alone and does not require any treatment .

Yes, it makes sense that it would be due to the accident.
The darkness pigmentation on the sac is on the right side only, which is the same side I had my injuries. However, my penis has had dark colored patches in areas over the years.
I am not sure what you mean my "swelling" of the sac. I was in intensive care for 3 weeks during recovery and I didn't notice any abnormal swelling or soreness. I did have a catheter in for few days.
As I mentioned, in the mornings, the top part of the sac feels hard not pliable, but later in the day, that disappears. That has been the case for a few years now.
One more thing I wanted to let you know.
I have noticed little black ( or dark red ) dots dispersed around the bottom of the sac along the veins. Upon closer look, they look like little blood clots, the size of a pin head. Not sure why they are there. They are not raised and very small.
Thank you for your suggestion to not use the OTC fungal cream, as this does not sound like a fungus.
Black spots on bottom of sac may be clots in superficial veins of scrotum.

The dark spots at the bottom of the sac may be capilary hemangiomas and nothing to be worried about it .
The hardness of the sac in the morning is mostly due to pulling up of testicle in the morning when the atmospheric temperature is little cool .This causes contraction of cremastic muscles to pull the testicle up in sac ,However this it has no significance at all.
