Suggest Remedy For Dementia In A Chain Smoker

Question: Dear doctor,
I have a issue with my best Fren of 27years, at the same time my business partner. He is 36 years old .During recently years of working with him, I realize he had some problems doing simple matters. He is constantly forgetful, however he never really think he had forgotten anything until u describe or rewind the situation with him CLEARLY! He could have just passed u a set of keys now and completely have no impression of it later. And he can swear with his life that he hasn't done that earlier! This is specially frustrating to work with someone like tat as he couldn't even handle the simplest task. We had tried several methods including writing down and making notes , he could have wrote 5 pointers and when he read he only read until 4 pointers and missed the 5th! You can ask him to buy a packet of noodles and he come back with a packet of rice . And he REALLY thought so!and he is constantly making mistakes because he is always telling u he forgot, hear wrongly, see wrongly , say wrongly and everytime his mistakes cause some negative impact he will feel bad for awhile and continue his problem the next day.

I'm writing in as he couldn't even describe his problem to you. He knew he had some problem but he can't relate because he don't realize how his situation had began to deteriorate.
He smoking heavily ( even at the busiest instances ) and needs to drink many cups of coffee in the day and drink beer to sleep everyday . As a his buddy, I'm seeing that his habit had gone worse, but he said that he don't feel it at all because it had always been like this. His addiction is far more than he knew.

He could have eaten burger the day before and swear that he hadn't had that for months !
Everyday with his weird behavior none of our colleagues can work properly with him. After talking to him he did recognise his problem but carry on his doings . I even tell him to video record the simplest instruction so that he can follow ( I mean I had to do that just imagine how my life is!)

He suspect it is inherited from his family as his brother and father is some sort like that too.
May I ask for your advice please

The first step is to get the cause of such abnormality at this age.
Following investigations are required like brain MRI, blood sugar , thyroid profile, vitamin B12, Renal and liver function tests.
Apart from these depression also causes memory loss.
Is there behavioural abnormalities like abusing, excessive talking or less social interactions.
Any other medical illness.
Any details about family history?
Treatment will depend upon results of investigations.
You can take him to a neurologist for detailed examination and investigations as advised.

Hi Dr XXXXXXX

There is no abusing in a violent way. But I did notice that he is smoking a lot more even though he denied it .. And he can walk long distance to buy the cigarette of his choice and away urgent work and responsibilities at that moment. He needs to drink alcohol to sleep and coffee to keep himself alert and awake in the day.
And his addiction is far more he can imagine .
Whenever he forget something or "assume " something and made unacceptable mistakes , he became very defensive and he can generate lies at that very moment just to cover up his wrongful act. He became clever for that "moment"
Because of his excessive cigarette coffee and alcohol , his breath is constantly unbearable smelly. And is getting from bad to worse. And as a childhood best friend I thought I should do something for him.
He is not married and doesn't had a girlfriend. He had a girlfriend but broke off several months ago because he felt restricted when with her as he cannot fufil his addiction.. He gotta lie to get out of the house to buy coffee and alcohol. Finally he said that the gal was not good n broke off with her.
Under my persuasion he went to the psychiatrist once and was diagnosed with dementia young onset as u said. But he never felt it was a problem as he thought every one looked at thing from a different angle. But the fact is that he is living in his own different world !

He did not have any medical history as I knew off. His father and brother seemed to behave quite similarly . The are generally financial stable selling food as own business.and im very sure they could never make it if they work for some companies .

Memory loss require detailed investigations as discussed earlier.
Timely intervention may lead to good recovery else the damage may be permanent.
