Brief Answer:
Please get cardiac and renal evaluation

Detailed Answer:
Dear sir , greetings from HCM . I understand your problems. You need to get your HbA1c, microalbuminuria done and the dose has to be adjusted based on that. Now the values are looking OK. Make sure that you are taking proper food after the diabetic medications.
Decide your meals in to 4 to 5 equal and small parts with 2 snacks in between.
You need to go for complete cardiac evaluation and renal function test to know the cause of leg swelling.
Regarding the nerve pain. Exactly you need to tell your physician what is the problem in detail. To rule out diabetic neuropathy.
He might advice inj. Neurobion as people on diabetic medications especially metformin will have B12 deficiency.
Proper diet, lifestyle changes, and adjusting the dose of medications will solve your problems.
Practice yoga and meditation to get rid of ur stess
