Suggest Remedy For High Fever Along With Severe Vomiting In Children

Question: Hi, my son aged 7 years old has developed fever to the measurement of 105 Fh since yesterday. He also had severe vomiting yesterday in the evening. I went to Pediatrician yesterday. He suggested paracetamol 500 mg every 6 hrs and in case fever does not subside then Maftal 250 mg. After 3 to 4 hours the temperature is rising to 105 Fh which I am controlling with mild sponging. What could be reason for this? I am awaiting the blood test report.

Welcome to the HCM

I understand your concerns but don't worry. High grade fever spikes with associated vomiting episodes can be due to various causes. The most common ones being:
1. Infections: enteric fever, malaria, urinary tract infection, dengue
2. Bacterial or viral meningoencephalitis
3. Occult abscesses
4. Inflammatory disease such as autoimmune diseases
5. Respiratory tract infections such as pneumonia

The list is long but blood and urine investigation reports may help us to get to a definitive diagnosis and thus the required treatment.
You may upload the reports for my reference so that I may help you out in the best possible ways.

You may continue Paracetamol and Meftal as advised for fever spikes.

Respected Doctor, Thanks a lot for your reply. I have attached the reports. I consulted the doctor with the reports. He said to me that he is more in favour of viral infection rather than Dengue fever even though the NS1 test reports suggest so based on DLC. He asked me to start tab Zifi 200 mg since today if the frequency of fever does not reduce. Since yesterday the fever frequency and temperature spikes have gone down. Fever is reporting after every 6 to 7 hours and is remaining around 102 Fh. I have started with tab Zifi 200 mh twice a day and tab Calpol 500 mg after every 6 hours. Also Giving my son plenty of fluid and food. However he is feeling quite tired. It is 4th day of his fever. Doctor, please suggest me if I am on right path of treatment. My doctor suggested to get PCV and Platelates count tests by tomorrow.

Reports suggestive of bacterial sepsis with dengue

Hi

The NS1 report is positive for dengue. But high CRP is in favour of an associated bacterial infection as well. So, I do agree with the treatment suggested by your doctor. You should continue Zifi 200 along with plenty of oral fluids. Do recheck PCV and Platelet counts to rule out any worsening.

Don't worry as most cases of dengue improve with out any complications. The only precautions needed are plenty of fluids orally and avoid ibuprofen for fever episodes.Also, bacterial infection improve with the course of antibiotics as suggested.

Ensure regular healthy diet.

Dear Doctor,

Thanks a lot for your timely and very useful advice. My son has not recieved any fever since yesterday evening. However, he is feeling quite tired. I got his Platelates counts and PCV checked yesterday and they were 1.33 lakh and 33.6, respectively. And I am continuing with Zifi 200 for another three days. Kindly let me know if I need any other precaution. When will the DLC parameters will turn to be normal?

Hi
Most parameters improve at the end of 1 week in cases of dengue. Since your child seems to be in recovery phase of his illness, his blood parameters should hopefully improve in next 2-3 days.

There is no need to go for further blood tests unless and until he gets back any symptoms. Avoid giving paracetamol unless and until he gets fever spikes. This will help to look at response to treatment.

Continue antibiotics as advised and ensure regular healthy diet with plenty of fluids.
