Question: Hi I am on xarelto for life due to factor V Leiden. I suffered a massive saddle PE 11 mos ago. 2 days ago I fell on the ice pretty hard And landed on my upper back and head. I don't have any bruises and have a sore back and headache. do I need to go to my doctor? Some lightheaded at times. Thank you.

I read your questions carefully and I understand your concern.

Xarelto as a blood thinner can make you more prone to bleeding in case of trauma. That risk varies also depending on other factors like: age, high blood pressure, liver or kidney disease, alcohol use etc. You don't give much data about those other factors, if you are a young individual without any other bleeding predisposing condition the risk is much lower.

After a fall such as you describe it is to be expected to have a sore back and some headache so in the absence of other symptoms I wouldn't panic and hurry to the doctor for the moment. Also since it has already been two days the risk is very low as hemorrhagic complications usually happen during the first 24 hours.

However since as I said in your case, taking a blood thinning product, the risk is higher I'll mention some manifestations which warrant medical evaluation like:

-Intense headache or headache which is progressively growing in intensity
-Vomiting
-Confusion, behavior changes
-Marked or progressively worsening dizziness with difficulty walking and standing
-Drowsiness
-Weakness of a limb
-Seizures

If any of those are noted you need prompt medical evaluation.
