I have read your query and can see that despite a month of taking 60mg prozac you have no relief from OCD symptoms.
I would like to tell you that you need not be dishearten by this as I have seen the similar scenario with my patients of ocd...they take more time compared to cases of depressive or anxiety disorder, in getting better.
In general we allow the medicine to continue for 8-10 weeks to have a response in OCD.
OCD is slow to show improvement. Even the dosage needed by an OCD patient is slightly on the higher side...
Had you been under my treatment I would have waited for another month before increasing the dosage further.
Please have faith in the medication...and dont take stress...the urges of harm are there but you will not act on them. They are troublesome but you dont have the compulsive component in you.

I can understand your concerns. It is indeed tiresome to feel on the nerves all time. But this will get better...dont worry.
As far as my clinical knowedge is concerned, I hope that you should feel improvement in a month more.
Till then if the stress disturbes you a lot- you may take etizolam 0.5mg on pnr basis. This molecule is very helpful in controlling anxiety and is not sedative at the same time. I have been using it in my clinic with my ocd patients quite regularly, with good response.
Prozac is a recommended drug for managing ocd- so, its just the matter of time you & you will start getting relieved.
Please have faith. Try to be busy with somework and be with people ..dont be alone.
Keeping company and and doing some work always helps in recovery.
If there is limited scope of socialisation in your area, then pursuing a hobby like gardening or just watering plants or cooking or even painting would help.
So, it will be wise to wait for a month more before increasing the dosage of prozac.

Hello,
I know it is disturbing for the patient...and even the treating team is concerned till there is no relief..but please wait..the medication will work, we need to give it another 4 weeks before we hike it further.
The medicine usually reduces the intensity of urges and then the frequency is decreased and finally the urges dissolve completely. Sometimes the sequence may change and the frequency if the urges is decreased from the start.
So, dont worry.. in any case, the response will be there.
You may ask your doctor to either hike latuda or change it . The role of antipsychotic in ocd is for potentiation of anti anxiety drug...if I am treating a similar case and there is hardly any benefit, then I usually go for increasing the dosage or shift to another molecule.
Something like resperidone or quiteapine have been researched to give good response. Even I have seen very good results with them in my ocd patients.
So, you may discuss the points with your team there and see if something can be done. The doctor will do whatever he thinks will be best for you. Please trust and have patience. You will get well.

Yes, in ocd there can be false imaginary images or scenes which are disturbing for the patient. Like, one might feel that he killed someone while driving or in your case as if you threw your kids ....but they arent true...those are just false or rather obsessive thoughts / imgaes which one might have in ocd.
They are repetitive in mind and person keeps on checking if he did so or not..
So, even your "false memories" are part of the illness and not delusions. I have seen them in many patients and I can understand how crippling they can be. But the medication helps them too.
