Suggest Remedy For Mild Fever, Joint Pain And Flushed Face Caused By Fifth Disease

Question: My sister is 62 years old, very healthy and active. Ten days ago, she had a flushed face, low grade fever, and joint pain in her fingers, wrists, and shoulders. She went to a doctor who diagnosed fifths disease and gave her Valtrex and told her to take Advil. Her pain has gotten worse. She can't do anything with her hands. She can't drive, she can't hold the leash to take her dogs for a walk. It has been 9 days and the pain is not subsiding. Any ideas?

I have gone through your query with diligence and would like you to know that I am here to help. To begin with, I would first like to go through all the symptoms that your sister is exhibiting to at least understand whether or not the diagnosis is accurate or at least close to being accurate. So please do write back to me with a yes or no to the listed symptoms, or elaborate on any if you have to:

+Joint pain which can last weeks even whilst on the treatment (main joints affected are wrists, hands, ankles and knees) - The most important symptom in adults
+Rash on the cheeks is common among children but not adults, but if it at all it is present please let me know(please also try to attach the photo to our discussion or email it to me at: YYYY@YYYY ; and please mention 'ATTN: Dr. Shoaib Khan' int he subject line so it can be directed to me easily)
+Fever (Rarely seen)
+Headache (not always seen)
+Stomach upset (not always seen)

I would like to also highlight that the pain your sister is experiencing may last for longer, although she is on treatment. This is a distinctive presentation for adults having Fifths disease, and unfortunately it does seem like she does have it.

Treatment also is adequate for the time being. Individuals with Fifths disease also develop anaemia (to be checked and blood transfusions need to be administered if required), and immune globulin injections (which help with the immunity). Immune globulin injections help a lot by improving the individuals immunity and thus killing the virus sooner. Please speak to her doctor about this.

But in my opinion, the pain may persist this long and may even last a little longer, unfortunately she will have to endure, I am sorry about that.

Thank you for your response. The face rash only lasted a day or two and made her fade look flushed. The fever never went above 100 degrees, and it too only lasted a couple of days. The joint pain and swelling is the worst part. She is a dentist and has had to cancel all of her patients! She has an appointment this afternoon with a rheumatologist so hopefully she will get confirmation of the diagnosis. Should she just rest or is walking good for her?

I would recommend the second opinion to confirm the diagnosis, thats a great idea. But would recommend against walking, as the knees can also be affected with Fifths disease, and any physical strain can only make her feel worse, so rest is best.

Please do keep me in the loop and let me know if the diagnosis was confirmed, and if it is then speak to the rheumatologist about the immune globulin injections to help boost her immunity.

Follow up: Dr. Shoaib Khan (47 hours later)
The rheumatologist took 8 vials of blood to test for a few different viral infections, including Fifths disease and Epstein Barr, but we won't get results until Monday. She also prescribed Celebrex. The pain persists, maybe even worse the last few days. She isn't sleeping much, can't move her right hand at all, her shoulders hurt, but so far; her knees have been okay.She says the pain is a 9 out of 10! She says Advil doesn't touch the pain. :(

I am sorry to learn about her current status, and your doctor should have really prescribed something more strong for her sake. Celebrex is a non-steroidal anti-inflammatory drug and should help with the pain, but with her pain being at 9/10, celebrex also will not do much to make her feel comfortable.

Two more days until Monday, and I do feel she needs something more strong to help her at least reach a 3-4/10, so if you do not mind please do speak to the rheumatologist or her primary physician to help provide her with something a little stronger so she can at least get a good nights sleep until the results come in on Monday.
