Suggest Remedy For Pain And Bleeding With Blood Clots

Posted on Mon, 29 Sep 2014
Question: I was curious.... 88 yr old father (with enlarged prostate) had urethra foley catheter inserted. Immediately voided large amount of urine. Then abdominal pain started and bleeding with blood clots. Nurses inserted and re-inserted the same foley catheter. Blood and blood clots continued. Next day urine was leaking from penis with foley still inserted. Doctor removed and re-inserted the same foley. urine collection in foley bag remained red from blood. Doctor told me that was normal. Father was discharged from hospital..... and re-hospitalized a few days later with blood coming from tip of penis. Found out foley balloon was inflated in prostate.

Is it normal for nurses and doctors to insert and re-insert the same foley catheter without changeing it for a new one? Is it normal to have blood and blood clots immediately following insertion of foley catheter? How does a foley catheter balloon get inflated in prostate? Can the inflated foley balloon be pulled out of the bladder?
In addition..... father now has chronic yeast infections in his urine that don't seem to go away. How could father have yeast infections instead of baterial; infection.... especially since there was damage to urethra and prostate?

Brief Answer:
Beeding may be from injury to Prostaic Urethra.

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that your father who had retention of urine due to Enlarged prostate and was catheterised to drain his bladder which was followed by a gross bleeding .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Since how many days he had retention of urine ?
2) Was he having dribbling urination for couple of months ?
3) Was he having pain in abdomen due to retention of Urine?
4) Was he having sensation of fullness of bladder before he was catheterised.?
5) Is he diabetic?

The bleeding that he had may be due to one of the two following reasons
1) The bulb of the Foley catheter is inflated with its tip being in prostatic part of urethra causing injury to urethra. or
2) Due to sudden release of pressure of bladder which was distended for a long time due to chronic retention of urine. The sudden emptying of bladder causes release of pressure over blood vessels which get ruptured and start bleeding (What is called as rebound bleeding).

Answer to your queries are

1)It is not common to inflate the Foley bulb in prostate but can happen accidentally which leads to such events .

2) Ideally catheter is to be changed and replaced with new one instead of using the same one to avoid infection.

3) Foley catheter can not be pulled out of urethra with inflated bulb if it is filled with 10-15 ml of water..

Please get routine urine test and urine culture done to find out the organisms causing this infection and antibiotics to which they are sensitive to.

Please take broad spectrum antibiotics like Cefexine along with urinary antiseptic like Nitrofurantoin twice daily and anti fungal medication twice daily.Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water. To keep his urine dilute This will help to control dysuria.

Hope I have answered your query ,please feel free to ask if you have more questions ,
I shall be happy to help you
Thanks and Regards.
