Question: swollen area above the pelvic bone on her left side , swollen area about the size and shape of a pear. this is on a friend of mine ... when she coughs it hurts a little ....

Noted the history about your friend having a swelling about the size of a pear on left side of the pelvic bone which hurts on coughing.

If you mean that the swelling is on the inguinal region, this is probably a hernia or a lymph node.
If the swelling is on some other area than inguinal region along the''pelvic bone'' (is all along the pelvis) it may be lipoma or a sebaceous cyst.

what is the inguinal region?
.. the area is on her left side above the pelvic bone about 1 inch from left side from the highest part of the hip...

Most probably lipoma or sebaceous cyst.

Detailed Answer:
Inguinal area is the one of lower abdomen near the pubic bone (center), and just above and by the side of the private parts.

The area you have just explained, may have lipoma as the most common reason of a lump, other causes can be sebaceous cyst and from the muscles or so.
