
Suggest Remedy For Persistent Night Cough In An Infant

Question: Hi, My daughter is 2.5 years old(DOB 23rd XXXXXXX 2013), and she has developed night cough since XXXXXXX 2015, earlier it use to occur once a month but the last one month she has been having this every night, Doctor has prescribed Asthalin and Nebulization with Dualin, It helps for sometime but after a couple of days she develops the cough again, and when she develops a cold she snores and breathes very heavily through the night, We have tried giviing her honey, steaming but this seems to frequently happen and cough hasnt subsided for the last 1 month, Any suggestion on the next steps? and what could be the possible cause for this

By what you quote I feel what your kid could be having viral associated wheeze or multi triggered wheeze. I have a few questions for you -
Questions:
1. How many days per month does she cough or feel breathless?

Answer: Until October she had once or twice a month which lasted for 5 to 7 days XXXXXXX but november has been bad she has had this cough almost for the last 3 weeks sometimes more sometimes less

Answer:5 or 6 days in a month

Answer: Never she has had problem of breathless, she is very active and never breathes heavily when she plays or runs

Answer: Looks like seasonal, It started off in XXXXXXX am from XXXXXXX the weather is a bit cold and dry so not sure she was absolutely okay until april
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
Answer: We do not have any family history of Asthma, I used to have dust allergy in my younger days but not later
6. Is the cough always associated with fever?
Answer: there has very rarely been fever for her, with cough, she is absolutely normal when she has cough no fever no cold, only in night she has heavy breathing and cough which lasts ffor 10 or 15 mins every 2 hours

Hi...By what you quote and the symptom analysis I feel that your kid might be having a multi triggered wheeze. She will require a short course of metered dose inhalers for this. I suggest a duration of 8 weeks and reassess the kid.
Suggestions:
1. There are certainly alternative management therapies in allopathy now-a-days. Medicine has advanced a lot and not asthma is 100% controllable.
2. Inhalers are the newest management strategies for this. If I were your paediatrician I would have suggested the use of Budecort metered dose inhaler (100mcg) 2 puffs twice a day through a spacer and this is for regular use for 8 weeks. Another metered dose inhaler is Levolin and this can used as rescue therapy whenever the kid is having severe cough in spite of regular usage of Budecort. The technique is very important and very crucial for the drug to be delivered correctly to the lungs. Regularity of medicine usage also matters a lot. So do not discontinue abruptly after you notice some improvement. The technique of administering an inhaler using a spacer has to be taught to you by your doctor and these are prescription medicines. So I suggest you consult your pediatrician for this.
3. Triggers can be environmental changes/ dust/ talcum powder/ seasonal changes/ un-cleaned a/c vents/ cold weather etc....we can specifically say this is the cause - unless we observe the kid closely - best person is the parent.
