Suggest Remedy For Severe Hair Loss And Hair Thinning While Having Seborrheic Dermatitis

Question: Hi Doctor , I have hair lost past 2 year , hair thinning in the crown region . For the past 6 months i have consulted a doctor , He prescribed me HAIR4U (Minoxidil 5% + Aminexil 1.5% ) and Ketafung - CT shampoo (ketaconzole and coal tar) and tablets like "androfol - M" , "Deng-M" and "Bio plus Forte" . I am using it continously . Now i see lot of flakes on my scalp (seborrheic dermatitis) and there is lot of hairfall when compared to earlier . But minoxidil has improved my hair texture . But i am really confused if i stop minoxidil will it have negative impact(will lose my hair). Please do help me , i am really worried.

Hello. Thank you for writing to us

I have gone through your query and I have understood your concern.

Minoxidil is constituted in an alcohol base therefore it can cause dryness and scaling on scalp skin. Since you have lots of flakes therefore I sugest you to temporarily discontinue minoxidil solution and instead use a topical steroid lotion (e.g topical 3% solution), on scalp skin, once daily at night, for a few days (i.e 10-14 days). This would take care of flaking. Thereafter, you may use a different preparation of minoxidil which has a less concentration of alcohol e.g androanagen solution Or alternatively you may reduce the frequency of application of minoxidil solution to once every alternate days. This would work good to maintain the hair density.

Thanks for your reply doctor.

One Doctor has prescribed me Betamethasone lotion i.p (Dipnate) and Folvite tablet (Folic acid) and other doctor has prescribed "Clonext's Lotion"(Clobetasol Propionate & Salicylic Acid Lotion ) I am not aware of what is steroid lotion in these two ?Can you please help me and can you please prefer me some brand of Minoxidil which is free from alcohol or which has less content which produces such flakes .

And i am undergoing Microneedling two week once (1.5mm) by Hospital staff , is it safe ? I have undergone mirconeedling more than 20 sittings . Thanks inadvance.

Both of these lotions, which you have metioned, are steroid lotion and you may use either of these for 2 weeks. This would be enough to resolve flaking. Thereafter you may use 'Androanagen solution' by curatio pharmaceuticals, which is alcohol free minoxidil.

Yes, microneedling is safe procedure in the hands of hospital staff.

Thanks a lot :) People say that usage of Minoxidil and androfol-M tablet may cause impotency , i am just 22 years old and not married yet. Will it have any adverse effect ?

Doctor has adviced me to use Minoxidil 1 ml both morning and evening, But i find 1 ml is insufficeint. Will it have any negative effect if i apply more than 2 ml/day ?

After 2 weeks of tropical steroid solution , i shall make use of minoxidil prescribed by you . Shall i use it on alternative days or shall i use it regularly ?

There is no long term effect on potency. Only 1-2 % of patients may notice a decrease in libido at start of therapy but this usually subsides with continued treatment.
Yes, you may use 2 ml minoxidil, once daily at night. This also ensures compliance in most patients.
Yes, you may use minoxidil solution on alternate days during maintenance phase i.e after the initial 6-12 months of regular minoxidil use, to maintain the hair improvement achieved.

Hi Doctor ,

I have few other questions.

One of my friend has "seborrheic dermatitis" i used his comb once unknowingly is that is the cause of the same in my case or usage of minoxidil with alcohol has caused "seborrheic dermatitis" to me ?

Can "seborrheic dermatitis" be completely cured by applying tropical steroid solution ? If so how many weeks or months it will take ?

Has minoxidil with alcohol has aggravated "seborrheic dermatitis" it more ? Can i apply alcohol free minoxidil with tropical steroid solution or shall i wait until "seborrheic dermatitis" it gets cured

How frequent can i take head-bath using ketaconzole shampoo in a week ?

Hi.

Seborrheic dermatitis Or dandruff (in common parlance) is not contagious and therefore cannot be transmitted by sharing a comb.
Seborrheic dermatitis is endogenous and is due to a combination of factors like fungus overgrowth (i.e yeast malassezia furfur) and inflammation.
Minoxidil may exacerbate seborrheic dermatitis due to its alcohol base Or induce a mild dandruff/ flaking in those who did'nt had it earlier.
Topical steroids being antiinflammatory are rapidly effective in controlling and resolving seborrheic dermatitis, however, seborrheic dermatitis may relapse after seemingly complete cure once steroids are discontinued. Therefore an antifungal shampoo e.g Ketoconazole shampoo is the best remedy for maintaining remission of seborrheic dermatitis once significant improvement has been achieved with topical steroids, usually within 2 weeks. Antifungal shampoo has to be continued indefinitely while you are also using minoxidil and also if you had underying seborrheic dermatitis
I would suggest you to discontinue minoxidil for 2 weeks and use topical steroid lotio, instead. Thereafter you may begin with an alcohol free minoxidil and also continue using an antifungal shampoo regularly for scalp wash.
You may use ketoconazole shampoo every alternate days Or even daily depending on the severity of flaking and response to treatment.

Does hair lost by dandruff can be gained back ? because i lost a plenty in few months . Is it a natural process ?

will there be any better growth if i completely shave my hair and apply minoxidil after the treatment of dandruff ?

Hi Doctor ,

Can derma roller be used when i have Seborrheic dermatitis ? Will it aggravate ?

If i have to apply both minoxidil and tropical steroid solution , which should i apply first ? and after long time can i apply the other ?

I can see the flakes after applying tropical steroid solution and after taking bath , it is stuck to my hair , can i put comb on wet hair to remove those flakes ?

How much drops or ml of tropical steroid solution should i apply ?

Hi.

Ideally deramroller should be used once all the inflammation i.e dermatitis has resolved.
You may apply topical steroid lotion first and then followed by minoxidil after 30 minutes to 1 hr.
Topical steroid would gradually resolve the inflammation and with it the flakes. There is no need to forciby remove the flakes with a comb. However, you need to do daily regular shampoo and this would be enough to remove the scales.
Approximately 1.5-2 ml of topical steroid for the whole of scalp would be enough.
