Suggest Remedy For Stress On Lower Back And Shoulder

Question: kidney discomfort vs skeletal muscle pain
GFR was great/Optimal back in September .
At my job I work with a lot of weights bending over 10-70 times a day pouring 40lbs of petro while in a awkward positon. I am also a avid bodybuilder and lift weights and run 4-6 times a week.
I have had a sublaxation of my ribs around July of 2014 and costocondritis. It has since passed
I have injured my back slightly 8 months ago doing deadlifts. Upon my normal routine working I added a new exercise that put a lot of stress on my lower back and shoulders ( Snatch and overhead press while standing). I noticed a few days after the workout that I had slight discomfort in my lower right side of my back and that I had some soreness/tenderness in my right flank only to the touch. The ache sometimes comes on the left side. It also feels like I have strained my groin area aswell. My urine is normal, no bowel problems, no fever, no chills no other symptoms and my pain is costant unless I am sleeping.. I have tried lortab and muscle rexlants and muscle relaxants seem to dull the pain by about 20-25% for around a hour and a half to two hours. Also heat seems to help a lot. I have been to a chiropractor who seems to think I have a muscle strain

Brief Answer:
Might not be related to the kidney
Detailed Answer:
Hello
Thanks for getting back
Since there is a strong history of lifting of heavy weights it is unlikely that the pain is arising from the kidney. Pain arising from the kidney is most often in the flank region and it is dull aching. Trauma to the kidney almost always leads to blood in urine ( which you have not noticed).
Here is what I suggest
1. Disc prolapse is what I am suspecting, get an X ray of the spine done first.
2. If that is normal get an ultrasound of the abdomen and renal function test.
3. Continue to apply heat
I hope I was of help, if you have any further queries please get back to me
Regards

Hello
It is possible to get dull ache from exercise, muscle tear is one good reason for the dull pain.
Symptoms of the kidneys are dull flank pain associated with fever and blood in urine. Some individuals also start developing puffiness of face and legs.

I took XXXXXXX 350mg, one at 10am and one tonight at 6pm.. seems to have helped.. third da y of taking them. No fevers or blood in urine that I can see.. the ache is hard to discribe
does a normal GFR, creatinine and BUN change drasticually over say 5-6 months
one last question. This started around a month ago when I though of the intial injury. I have taken cipro for 8 days few weeks ago but it didn't seem to help and hurts a lot riding and right after moving around a lot.. during the night I notice no pains.. sorry for the rants.

tab XXXXXXX is a potent drug which is also nephrotoxic, it can occasionally lead to renal failure when used for a prolonged period of time. I recommend that you stop using it and take acetaminophen instead.
I pain does not look like it is of renal origin , since it's been a long time now I recommend getting it throughly investigated without delay. Taking painkillers only gives symptomatic relief but does not take care of the primary problem
Pleas get back to me if you need help with set of investigations for the pain.
Regards

a follow up question doc. I have read that after a over night fast, sediment in urine left standing in a container in the refridgerator will cause a cloudy sediment if left for more then 8 hours shows that the kidneys are filtering very good. Is that sediment normal?
I also read that after the fast has been broken by water, food, etc.. that the filtering sediment will stop and samples placed in the refridgerator would be clear.
is any of this true?

Hello
The most common reasons for urinary sediment is proteinuria ( loss of protein in urine) which can occur in traces after fasting ( prolonged fasting not overnight) and exercise, urinary tract infection and kidney stones.
Filtering of the sediments is a continuous process, it never stops.
I hope I was clear in my explanation. Please get back to me with any further doubts if you have, I will be happy to get back

so to ask another question if one does this, say urinate into a container first thing in the morning and let it settle in the refridgerator and leave for 6-8 hours and upon checking its cloudy that is a bad thing?
but the subsequent urine samples throughout the day are all clear after lingering in the refridgator for 6-8.. this only happens in the morning after a night of sleep and maybe urinating once during the night

Brief Answer:
Improper sample collection
Detailed Answer:
Hello
I am sorry for the short delay in my reply, it was a very busy day at the hospital
Early morning sample is always voided out and the sample what we take is the second void of urine simply because the sediments will be high in the early morning sample due to over night stay in the urinary bladder.
If your urine routine is normal in the second sample of urine then it is a normal urine sample and there is nothing to worry about
I am once again sorry for the short delay

Thanks doc, this will be my last question.
My chiropractor did a murphys test (sign) where he punched the kidney area in my back on both sides multiple times with no pain on the left and none/only slight (scale of 1-10 maybe a half) tender spot on the right but he did find a small area upon pressing near my spin on the right side where he massaged and noted my left side was tight and after the adjustment. I felt great for about a hour or so. Is this normal with muscle strains?
The pain is a 5 on a scale of 1-10 and its a more of a nagging, ach then anything else and it hurts more now then before the adjustment. I have had strained muscles in my shoulder, and back before and this is different, feels more like gas/ach then a true dull or sharp pain.

Brief Answer:
Muscle ache
Detailed Answer:
Hello
Thanks for getting back with these details, it adds a lot to the clinical decision.
The muscle next to the spine is erector spinae muscle which does get affected by lifting weights, which also explains why muscle relaxants are working. Kidney related pain will not reduce with muscle relaxants.
It is time you cut down on your weights to the ones you dont get affected by
Regards
