Question: my room mate suddenly developed extreme pain in the bottom right section of abdomin. he had cloudy /bloody urine last night, 45 year old male

Hi.
Thanks for your query.
Read and understood history about your room-mate male/45 suddenly developing pain in the bottom right section of abdomen.
He has cloudy/bloody urine last night.

With this history the first diagnosis that comes to mind is the impacted stone in the right lower ureter with urinary tract infection.
The possible other diagnosis are appendicitis, colitis and so on.

I would advise him the following:
To go to ER. Will not go to Doctor will not help as he needs to be evaluated and needs prescription based treatment that you can get from a Doctor alone.

First of all to give the urine sample for routine, microscopy, culture and sensitivity before taking any antibiotic to get a better result.
X-ray of the abdomen for KUB.
Ultrasonography.
Tests of blood.
This along with clinical evaluation and examination will give the probable diagnosis and a plan for further treatment and investigations if any.
He shall be started on antibiotics and medicines as per the symptoms and signs.
