Question: Hello. I have recently felt a small (very tiny) "bb" like knot at the top of my left testicle. It doesn't hurt but is slightly tender to the touch. I am 45 year old male with no health insurance. Any ideas what this could be?

Brief Answer:
looks to be an infection and needs prompt treatment!!!

Detailed Answer:
Dr. Hanif warmly Welcomes you to my virtual clinic HCM!
I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

Human anatomy of this region where you have an issue:-
There is a coiled tube situated behind and above the testes and the function of the tube is to store and mature the sperms made by the testicles. This is called Epididymis.

If due to any reason there is an infection of this tube (Epididymitis) it causes such tenderness, sometimes swelling of the testes or scrotum depending on the severity.

It sometimes is also associated with prostatic enlargement in males above 45 years of age. Urinary symptoms are also seen then along with local pain and swelling.

Management:-
If I was your treating physician I would have prescribed you with:
Azithromycin if not allergic to.
Ibuprofen.

You need to take proper rest and plenty of water.
Observe the barrier contraception (condoms) to avoid the spread of the infection to your partner.

Have a wonderful time ahead. Hope I answered all of your questions. Let me know if you have any further clinical concerns or doubts. I will be more than happy to get back to you in no time. Thank you. Does this mean without prescription, that rest and ibuprofen is the best place to start treatment since I have no prescribing Dr.?

I know you can't rule out testicular cancer, but I am assuming that your best guess is that it's prostate enlargement or infection and not TC?

Is there anything I can do to prevent prostate enlargement?

Brief Answer:
It is an infection and I think antibiotic is necessary if possible.

It is better to try for antibiotics. But if not possible in anyway, then yes, you should start with rest and Ibuprofen to relieve inflammation to some extent. If infection will be in preliminary stages and gets recovered without antibiotics; and, there does never come the urinary tract infection, then you may not need the antibiotics perhaps.

It is infection. Neither a prostatic enlargement nor a cancer. You have no urinary symptoms so prostatic problem is unlikely. I was just giving you the ideas that why this infection of Epididymis may occur.

Prostatic enlargement can't be prevented. It is very prevalent and is a benign disease not carcinogenic. You don't look to have this disease.
