Brief Answer:
Increase levodopa/carbidopa dose.Levodopa sustained release.

Detailed Answer:
Hi xxxxxx,
Thanks for being on healthcaremagic.com.
I am Dr.Ajay Panwar,a neurologist,here to answer your query.

Apparently, your parkinsonian symptoms are not responding to the present doses of Levodopa/Carbidopa and Requip.

I suggest you to please discuss with your treating neurologist regarding increasing the dose of carbidopa/levodopa(syndopa),increasing the frequency of syndopa and adding controlled release preparation.
Other drugs like MAO-B inhibitors may be added.
Deep brain stimulation may also be planned,if suitable clinically.

If you have any further questions,I shall be glad to answer.
