Question: I have been taking 325 mg of aspirin to prevent DVT due to a positive lupus like inhibitor blood test. I have had 2 more blood test to check the lupus like inhibitor and they have been negative. Do I need to be on aspirin and if so what dose, is it dangerous to stop taking aspirin cold turkey? Can I stop aspirin cold turkey can replace it with nattokinase supplement?

Hello!

Welcome and thank you for asking on HCM!

Regarding your concern, I would like to have some more information on your symptoms.

Do you have DVT symptoms (leg pain or swelling) or Lupus symptoms (joint paint, face redness, skin changes, etc.)?

What other tests have you performed for the diagnosis of DVT or Lupus?

As your Lupus antibodies have resulted negative twice there is real little chance for you to suffer from this disorder, and even more to suffer from a Lupus complication like DVT.

From the other hand, based on the XXXXXXX Heart Association guidelines aspirin is not effective in the prevention of DVT in high risk patients for this disorder. Anticoagulation therapy is needed for this purpose.

So I would recommend stopping aspirin if the diagnosis of Lupus has not been confirmed and you have no signs of possible DVT (with a normal Doppler ultrasound of the leg vessels and normal blood coagulation tests).

There is no reason to take aspirin.

Regarding nattokinase supplement, there is insufficient clinical based data to support its effects on the prevention of DVT in humans. So I would not recommend taking this supplement (even considering the fact that the diagnosis has not been confirmed).

Hope you will find this answer helpful!

Kind regards,

Dr. Iliri

Detailed Answer:
Hi again,

I carefully reviewed your uploaded medical tests and would explain that they are quite normal, except for a slight increase of blood glucose level and glycosylated hemoglobin (HbA1c) level.

Your ANA, B2 Glycoproteins, Cardiolipin, LLI, INR are quite normal. No evidence of antiphospholipid syndrome or coagulation disorder is present.

So, relax!

There is no need for anticoagulation or anti-aggregation.

The only advice is to better control your body weight, as an elevated HbA1c level indicates increased risk of diabetes (HbA1c>5.6%) and lowering your body weight would avoid this metabolic syndrome and would lead to a better utilization of blood glucose levels.

I recommend following a healthy balanced diet, like Mediterranean diet and a physically active daily life-style profile.

Wishing you a pleasant weekend!

Greetings!

Dr. Iliri

One more question, is 81 mg tylenol just as effective as 325 mg as far as the effect on platelets? Can i just take the 81 mg for about 2 weeks and the stop, i feel more comfortable doing that.
