Brief Answer:
Hiatus hernia related GERD

Detailed Answer:
Hello,
I can understand your concern for symptom related to digestive system.
1) Discomfort in chest with previous diagnosis of GERD suggest possibility of episodes of esophagitis (inflammation in lower/middle part of esophagus) due to reflux of acid from stomach back into esophagus.
2) Level and severity of inflammation depend on how severe the reflux is. In worst cases, patients can also develop inflammation up to upper part of esophagus also.
3) Hiatus hernia is most common condition associated with long duration GERD.
4) GERD is medical condition and can be controlled with medications for acidity and medications to improve motility of stomach which reduce chance of reflux.
5) Hiatus hernia is surgical condition in which lax opening in diaphragm through which esophagus enter into abdomen is strengthened by surgical repair.

Hope I answered your query. If you have any further doubts, I would be happy to help you.
Have a nice day!
