Suggest Treatment For GERD

Question: I was diagnosed with GERD and Barrett's esophagus 3 weeks ago after an endoscopy and biopsies. My gastroenterologist put me on lansoprazole 30mg once a day. My symptoms have only been constant throat clearing, a dry cough after lunch and a full feeling in my esophagus that comes and goes and doesn't seem to matter what I eat. The cough has cleared up but the throat clearing and fullness are still there. Does it take awhile for the PPI to work or should I have relief by now?

Hello,

I am Dr.Senduran,General physician answering your query.

In my opinion,constant throat clearing and fullness in the throat favours the association of LPR(laryngopharyngeal reflux) component with the established diagnosis.You may need to get an ENT opinion if you experience any voice changes,persistent throat irritation.

LPR is actually the extension of GERD I.e reflux upto throat(sometimes even behind the nose).

Initial management would be more or less same in both the cases.
1)Encourage soft bland diet
2)Avoid fatty,junk and deep fried foods
3)Multiple XXXXXXX meals instead of large ones
4)Elevate the head while sleeping with 2 pillows.
5)Avoid going to bed after meals(recommended minimum 2 hours gap)
6)Avoid alcohol,smoking if you do and coffee too..
7)OTC antacids and mainly PPI drugs.

Usually treatment response starts to occur minimum after 8 to 12 weeks with proton pump inhibitor drugs(PPI) like lanzoprazole(once a day).
