Suggest Treatment For H. Pylori Infection

Question: Hi , I was diagnosed with H pylori and treated with triple therapy (2 antibiotics and PPI) for 2weeks in Feb. I was asked to continue with PPI for 1 month. While on medication I was ok. After the completion of course , I had lot of side effects like anal gas, abdominal and anal pain. last month or so things seemed to have settled down and I was feeling ok but now again I had bout of stomach pain with some acid flowing upwards. The pain is not very severe but light more like irritation but everytime I eat I have some irritation in stomach and some discomfort in abdomin. My feeling is that H pylori is not got eliminated fully since symptoms are similiar to earlier. Pls advise

I have gone very carefully through your query. Your problem seems to be due to gastritis or gastroesophageal reflux. Although H.pylori predisposes us to such a condition, it is not essential that the problem will resolve completely merely on eliminating H.pylori. In fact, it is more of a lifestyle problem. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep (fluoxetine) 20 mg tablet (long term anxiolytic and a prescription drug) once daily for at least 2 months (it may take time for the action to set in). Discuss about this with your treating physician.
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Stress reduction has a very important role to play. Yoga and meditation should help.

Yes I am prone to stress and anxiety and lack of proper sleep. In the past I have for brief period taken nexito 5 mg. I have made lifestyle changes in terms diet , walking and doing some yoga. Would stomach and abdominal irritation as be caused by stress. Would continuing with anxeity medications cause side effects

Stress and lack of good sleep are direct contributors to the gastrointestinal problems you have described. I do not think that you need to be concerned about possible side effects of anti-anxiety medications. I find the rewards to be much more htan the risks.

Dr: Are we completely ruling Hpylori relapse since I have heard hpylori causes a lot of symptoms when it becomes active

No, we are not ruling out H.pylori relapse completely. In fact, H.pylori can recur. It is acquired from food sources and since the eating habits remain constant, the possibility will always remain. However, lifestyle modification is more important and needs to be implemented first.
