Suggest Treatment For IBS, Gas And Diarrhea

Question: Hi.
My husband is having stomach problem. Dianosis is IBS. He goes to toilet 3 to 4 times in a day. (Twice in morning and one or two times in second half of the day ). No stomach pain. However he gets urgent calls which makes him run to the toilet. Usually semisolid stools but sometimes lot of gases with loose motion. we did blood check up including sugar. Thyroid sgot lipid profile ceratine B12 and D3 etc. All tests are normal except B12 and D3. We have excluded wheat from his diet as it contains gluten. He is 38 and weights 77 (5 kg loss in 3 months. ) last year he had same problem. Tried many medicines. Got cured after 3 months. This year same problem at same time of the year. More than 2 months he is having this problem. Our family doctor says it is bcoz of stress and B12 deficiency. Would like to have second opinion.

Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.
I presume the weight loss was unintentional.
1. Did he find relief after excluding wheat? Was test for celiac disease done?
2. Was a small bowel biopsy done?
3. Was colonoscopy done?
4. Do bowel movements disturb his sleep?
Regards,
Dr.R.K.

Hello doctor
None of the doctors have advised these tests. Infact we had asked doctors if such examination was required. But they didnot recommended s7ch exam. As regards sleep some times he gets early morning. But he says he cannot sleep once he gets up and he doesnot get up becoz of bowel movement.
Regards
Weight loss is mainly bcoz he eats less than regular intake. now for last 3 weeks weight is stable as we have started bhakri in his diet.
He takes only one or two fruits in morning breakfast.
He is on strict diet no oily fried spicy food. No sweets etc.
Last year he lost around 6 kg and gained it once he stomuch is normal.

Brief Answer:
Answer given below
Detailed Answer:
Hi,
Since he is losing weight and is passing semisolid stools and is Vitamin B12 and Vitamin D 3 deficient, malabsorption syndrome should be ruled out.
The following tests should be done -
1. An upper GI endoscopy with small bowel biopsy
2. Tissue Transglutaminase antibody test
3. Colonoscopy
You can discuss with your gastroenterologist about getting these tests done.

Dr.R.K.
