Suggest Treatment For Irritable Bowel Syndrome
Question: Several years ago I was in the Bahamas and I got extreme pain in the stomach. I went to the hospital and the doctor gave me Baralgin 100 mg. it resolved my pain pretty fast. I now have gastritis, or IBS and was wondering if this medication would help reduce some inflammation? I see that this med was banned in the USA. Any reason?

Hi,
To my knowledge, Baralgin is a combination drug of metamizole and buscopan. Metamizole is banned in USA since 1977 due to causing severe side effect (agranulocytosis and other types of anaplastic anemia).

Dr klerida, I believe you have answered my queries in the past several weeks. As a gastroenterologist you may be able to answer another question. On my Barium test the radiologist reported that I have an organoaxial malrotation. My endoscopy did not show this malrotation. One of the doctors on this site explained that an endoscopy may not show this malrotation. The Barium test showed no obstruction with the malrotation. As you know I have been complaining of abdominal pain for 4 months and it doesn't seem to be healing. I have gastritis and possibly IBS, but this Malrotation is a mystery and I am trying to discuss this with my GI doctors! but haven't heard any answers as of yet. I had a Barium test many years ago and I do not recall anything about a malrotation. I understand that a malrotation is usually a birth defect, but can injury cause this to happen later in life, and could this be causing my pain?

Hi,
Before running into final conclusions, I'd ask to have the reports of your Barium test. Apart being a birth defect, it can happen after 50s.
I should understand first for what kind of "malrotation" your doctor is describing, then, to answer your questions the most correct way.

Thank you for sending in the reports. Although the quality of the image was not to the desired levels, I have noticed that the reports were negative for hernia or reflux.
The only findings were "Organo-axial malrotation", that most probably explains why you are having such problems.
Please bare in mind that malrotation is commonly happening in patients after their 50s. It cannot be shown in endoscopy; that's why Barium test is the specific test to diagnose malrotation.
If it has not be shown several years ago, this explains that it has happened recently (after your last Barium test).
Please bare in mind that when a patient comes to my office with such symptoms, the very first evaluations is through endoscopy and ultrasound. If nothing found, and symptoms persist, then, specific tests (like barium test) are carried out to determine the right diagnosis.

Dr Klerida, how could a malrotation happen at my age and what is the course of action to fix this? And, why would my specialist,by way of endoscopy, determine no malrotation.

The cause of malrotation at your age might be related to :
- previous trauma
- phrenic nerve injury
- abnormal ligament fixation
With regards to the treatment, it all depends on the symptoms. Usually, the chronic malrotation does not require any specific treatment unless there is:
- ischemia
- acute symptoms of stomach narrowing (strangulation)
As mentioned earlier, malrotation cannot determined by a routine examination like endoscopy. Barium test is the specific and the right examination for such problem.
