Question : Hello Doctor,

i've been suffering from mrsa folliculities which is causing recurring folliculities inside nose. This has happend thrice already and each time i took antibiotics.

However, each time i take antibiotics, it becomes ressistant to it. I'm worried that soon il run out of options.

Since folliculities inside nostril could be dangerous, im really worried. So is there a way to prevent this infection such as Electrocauterization of all blood vessels inside the nostril ?

What other options are there to permentanetly prevent this?

Brief Answer:
In my opinion , there are predisposing factors for your folliculities.

Detailed Answer:
Hi XXXXXXX ,
Thanks for the query.

I have read your query reviewed your medications and understand your concern.

In my opinion , there are predisposing factors for your folliculities inside nose like nasal allergy ,rhinitis & repeated itching trauma if any that need to be corrected .

If I were your treating Doctor for this case , I would advice you
Saline nasal spray thrice daily ,
Antihistaminic levocetirizine + Montelukast combination at night till allergy sunside ,
Application of sensitive antibiotic + steroid ointment twice daily for long term even after apparent cure since recurrent rate is high ,
& in your chronic case Electrocauterization or cauterisation with silver nitrate can be done .

Predisposing factors for your folliculities should be taken care of .
If you have long nails ,then they need to cut prevent fingernail trauma .

I would advice ,Consult ENT specialist in your city & get diagnostic nasal endoscopy done to rule out any rhinosinusitis .

Hope this information helpful to you .If you have any further questions I will be happy to help.
Regards.

Thanks for the prompt response.

Well the way it spread into the nasal area is i once touched inside the nostril after scrathing the scalp folliculities. So it spread to nose 3 months back. Ever since then i have recurring episodes of it.

I have done culture tests and it has shown to be Staph A.

I am going to follow up with a ent. Could you clarify the following

1. Does Electrocauterization or cauterisation cut all blood vessels in memcose membrance of the nostril 100% ? hypothetically if done, can i be sure that even if there is recurring folliculities in future, it wont have ant risk of germs spreading . What are the cons of this procedure ?

2. Does all nose folliculities cause spread of bacteria to inner layer > blood vessels or is it only the cause of celluties in nose? I mean what i get are small pimple like ones with puss filled.

Brief Answer:
Cauterisation will prevent further spread of nose folliculities

Detailed Answer:
Hi XXXXXXX ,
Thanks for the follow up query.

Electrocauterization or cauterisation will prevent further spread of nose folliculities & there will be chances of recurring folliculities in future. That is a con.
Crusting can result in nose due Electrocauterization or cauterisation .

No , all nose folliculitis cannot spread bacteria to inner layer only invasive bacteria can spread . There are many other cause for cellulitis in the nose .

In your case , i would advice you follow above treatment plan before going for Electrocauterization or cauterisation .

You will get relief .
