Suggest Treatment For PCOS When Trying To Conceive

Question: Hi doctor.
Myself, XXXXXXX from XXXXXXX I have been happily married for 3 yrs and now seriously planning to extend our family
Just to give you my background, I have PCOS and was having irregular periods earlier before i saw my gynae.
She kept me on Metformin-XR 500 thrice and then twice a day for increasing my ovulation chances.
I also took fertomid followed by my estra in one cycle. The same cycle we tried to have relation on my fertile days as suggested by gynae but with no results.
Last month i.e April, i was in fertomid again followed by follicular monitoring. The scans were normal and did show ovum rupture. We again made relationship on the said dates. My LMP was 11th April and now on 9th May i have started bleeding profusely with backaches.
I informed my gynae about this and she told me not to continue fertomid this month and make relations as usual.pls suggest

What is your age , XXXXXXX ?
Since when are you actively trying to conceive ?
Actually you have already had 2 cycles of Fertomid alongwith follicular monitoring, and you did have confirmed ovulation both times.
Now, for the next step -
1. First of all, please get a semen analysis for your husband . THis should be done after 3 days of abstinence at a reputed lab. Nobody should take medications like Fertomid without getting a semen analysis first, as it is a simple and most important test.
2. Then you can take this cycle of rest, trying naturally around probable dates of ovulation.
3. You can go for one more cycle next month of Fertomid with timed intercourse.
4. If all these 3 cycles fail, you can then go for Fertomid, and combine it with IUI ( intra uterine insemination ) .
5. If that fails, then please get a laparoscopy done to check if your tubes are open.
If they are blocked, then you might need IVF ( in vitro fertilization ).
In any case, remain active, healthy and stress free.
Take daily 5 mg folic acid also.
