Suggest Treatment For SLE In A 12 Year Old Child
His specific symptoms are
mild fever, ++ protein in urine.
no skin rash or anything else.
he is also given HCQS 200 I do not know which way is benefiting.
Is there a way to come out of this?

Well, Lupus in a young male is very unusual. I have read the other questions also you had asked previously and I feel we need a step wise approach here or the disease is going to keep progressing.
1. At times you need a low dose of steroids in SLE and it is never easy to come off it. IF the requirement for Steroids is continuously high, it is better to add a Steroid Sparing drug like Azathioprine or MMF. Considering his growing age, it is always better to keep the steroids on lowest dose
2. ++ protein in urine is not a good indication. I am not sure if you have got 24 hour urine protein done recently. If you haven't, you should repeat it soon, so we know the exact amount of leakage going on. If the leakage is high, I would suggest you consult your rheumatologist and he may need a kidney Biopsy done to look if lupus is involving the kidneys or not. This is a standard approach practiced world wide, but yes kidney involvement asks for an early intervention. There are good drugs available for it.
3. I feel he needs a step up of the treatment. What we need to understand is we need to hit the disease hard cause the maximum damage it can cause is in the first 3 years of it s starting. Better we control it now, better the results are.
