Question: hi,
I wanted to know how to identify MDR tb or xdr tb sooner .
my mom (58 yrs)has been taking trac 4 tablets from last 4 weeks. her MRI is due in Tuesday. but I wanted to know how all variants of tb can be diagnosed before hand so that treatment can be started as soon as possible.
I am more concerned about early detection of MDR,xdr or tdr tb
thanks

Brief Answer:
Based on efficacy of treatment.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney.
I understand your concern.
See, MDR TB or XDR TB is identified or suspected when a person is not responding to the standard treatment as guided by getting sputum positive for TB bacilli even after 2 months of appropriate treatment.
We also suspect that if a person has left the treatment previously in between or has got relapse.
We don't otherwise usually do workup for MDR or XDR TB.
However , if you are very concerned, you can simply get a Sputum Culture for TB bacilli and drug sensitivity done, which would definitely clear the picture. But ideally, this test should be done before starting the treatment.
I hope it helps.
If you have any further query please ask me.

Brief Answer:
Not required.

Detailed Answer:
Dear Friend
Routinely again , we don't do workup for MDR in cases of TB Spine ( Pott's Spine).
You would require her bone biopsy and Culture and sensitivity in case you want to know the sensitivity of the drugs.
This is cumbersome and not routinely performed.
I suppose, her chest Xray is clear.
Let her take the proper treatment, she will get fine.
If you have any further query please ask me.
