Brief Answer:
Needs to investigate and take appropriate antibiotic as per urine culture

Detailed Answer:
Hello
Thanks for query .

Based on the facts that you have posted your wife seems to have UTI which has not responded to Bactrim .

The UTI that she has is mostly secondary to what is called as Senile Bladder Neck Stenosis which is very common in elderly females .

Consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Endoscopic Examination of bladder (Cystoscopy) and Urethral Calibration .

In the mean while start taking Cefixime and Nitrofurantoin twice daily along with Urine alkalizer thrice daily and switch on to appropriate antibiotic as per culture report.

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.
