Question: Hi,Dr Rynne.about 2 Sunday`s ago I cut a huge hedge down (privet) took all day, once I had finished the weather turned really cold as I was bagging the hedge grindings down.always suffered with a lower back problem ,had a warm shower and got the wife to apply some Ibuprofen gel to my back, felt fine next day took some Ibuprofen tablets and applied more gel.all seemed fine ,then it all seemed to go pear shaped and I could not hold my urine for long then it went further and started burning so thought drinking lots of water may help only to make me urinate more,I have no pain but it is really getting up my nose as I cannot go anywhere and need to be near a toilet,I am drinking loads of fluid but have stopped with the pills and rubs ,could I have poisoned my self with privet or pills ,did not have a lot to eat that day but did consume a few coffee`s,early morning my urine is quite dark and stingy, hope this makes some sense .regards. M.Leggett ps i`m 62 pretty healthy and only just retired from a busy job(manual)

Brief Answer:
Urinary tract infection (UTI)

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns. Now it seems that your query was unattended for quite a while, so please allow me to answer it.

You seem to have urinary tract infection (increased frequency of urination, burning sensation, urine color dark). As you are 62 years old some degree of prostatic enlargement and bladder neck obstruction is not unlikely and hence prone to UTI . Please do Urine routine examination, culture and sensitivity test and Ultra-sonography abdomen.

If urine shows pus cells > 5 /hpf,a course of urinary antiseptics (e.g Fluoroquinolone group ) can be started pending culture reports.

Your backache is related to your hedge cutting over long hours and carrying hedge grindings etc. Ibuprofen , a NSAID (Non-steroidal anti-inflammatory drug) may have some averse effects on kidney on prolonged use.

Local application of short wave diathermy over small of back and analgesic gel(ibuprofen/diclofenac) application will work .

Orally you can take Tab Paracetamol 500 mg three times a day for 2-3 days +/- Tramadol for pain relief.
