A.2 Yes, I feel pain and swelling in anal region while passing stool.
A.3 No, there is neither loss of weight nor loss of appetite.
A.4 No, stool examination has not been done due to hesitation actually I don't feel comfortable.
A.5 No, any other investigation has not been done.
A.6 My diet is 2 chapati with Ghee, 1 bowl rice. actually I am afraid of abdominal pain so I don't take proper diet because according to my observation mucus is increases due to vegetables and fruits.

Dear XXXX,
Before attributing the cause of your symptoms to Irritable bowel syndrome , it is important to rule out local anal pathology. Irritable bowel syndrome is diagnosis of exclusion( If no cause is found for the given symptoms after thorough investigation then only it is attributed to IBD).
I would like to consider the following possibilities
1. Anal fissure: very much likely in view of history of pain while passing stools. Diagnosis requires good examination of anal region
2. Colitis
Vegetables and fruits are intact beneficial.But if you have observed any pattern of increased pain or symptoms with given food stuff then avoid them.
I would suggest you the following
1. Drink plenty of water everyday:. It will help by mixing with the fiber of diet and give bulk to the stools so that they are well formed and soft.
2. High Fiber diet: Vegetable salad( 2 servings a day), Fruits especially citrus fruits.
3. Avoid Diet causing constipation: Especially those food stuffs containing refined flour like cakes, bread, biscuits, cookies
4. Hot Seitz bath: Add a handful of common table salt in a tub of lukewarm water and immerse the bottom by sitting in it. Every such session should last for 15 to 20 minutes. Two such session a day, one in the morning and one before retiring to bed at night. It would also be benificial to repeat it after every bowel movement. It will help in healing of fissure, decrease pain and relieve internal sphincter spasm.
5. Local application of local anesthetic ointment like Xylocaine will help in relief of pain. Apply it 15- 20 minutes prior to every bowel movements and also before retiring to bed.
6. Nifedepine Ointment: Local application of it will relieve the spasm of internal sphincter and help accelerate fissure healing.( only after it is diagnosis has been confirmed)
7. Course of antibiotic: Ciplox TZ twice a day for 7 days

Pain while stool passing is not regular it happens only when I feel swelling in stomach but that swelling and pain happens 70% times. My problem is more concerned with mucus. Mucus in stool increases after swelling.

Also consider taking a Probiotic, it will take care of mucous discharge. Darolac capsule twice a day will be helpful. If you have developed intolerance to Ciplox TZ then discontinue use of same.
