Question: Hello, I am glad to talk to you. First, I will tell you the meds I am on. I have been on Celexa and Trazodone for years. In the last year, I have been put on Singulair and Dymista nose spray. I have acid reflux and a Z-line esophagus. I am afraid of this turning into cancer at some point. I also have Charcot XXXXXXX Tooth disease and cannot take Proton Pump Inhibitors. Can you help me with suggestions?

Recommendations detailed below

Detailed Answer:
Hello,
Thank You for the query at HealthcareMagic,
I have gone through your query and I understand your concern.

Since your endoscopy has shown Z line esophagus, the risk of barret's esophagus (which is the precursor stage before developing cancer) and cancer is minimal at present.

However, the key is to have regular endoscopic surveillance every 2-3 years to look for development of barret's esophagus. Barret's esophagus if caught and treated early will prevent progression to cancer.

Proper management of GERD is very important. Since you cannot take omeprazole, continue taking Ranitidine and follow these recommendations:

- Avoid spicy, fried, fatty, citrus food and chocolate
- Limit coffee intake
- Avoid smoking, limit alcohol intake
- Do not lay down on a full stomach. Give at least 2 hours between dinner and bedtime
- Raise the head end of bed using wooden blocks underneath bed post
- Eat frequent small meals rather than larger portions at a time
- Weight reduction, if needed
- Avoid medications that irritate the stomach. If it is necessary to take such medications like aspirin, Ibuprofen etc. take it after food only
- Most importantly regular follow-up with your doctor for endoscopic surveillance

Hope I was able to answer your question
Please feel free to address any more concerns

Dr.Lekshmi
