Question: I have had actinic poro keratosis for about 30 years. It is gradually getting worse despite my never going out in the sun and covering up when I do . Years ago I had treatment by freezing and applying specialist creams, but neither were effective. Is there any new treatment available now?

Disseminated superficial actinic porokeratosis, or DSAP, is inherited as an autosomal dominant characteristic. DSAP mainly affects the lower arms and legs. It has a characteristic clinical appearance of red-brown annular macules/patches which on close examination have central atrophy and a peripheral hyperkeratotic ring.
Unfortunately, there is no very satisfactory treatment for DSAP.
Traditionally cryotherapy/freezing has been used for larger lesions.
Other treatment modalities that have been tried are:
-5-Florouracil
-5% Imiquimod
-Vitamin D analogues e.g calcipotriol
-Oral retinoids
-Laser (Q-switched XXXXXXX Laser) etc
However noting has proved very effective long term
I would suggest that next time you visit a dermatologist for freezing, you can and get a prescription for one of these.
