Suggest Treatment For Acute Pain In The Lower Back

Question: Hi Doctor,

Please see my mother's medical history as stated in the link. She is taking levofloxacin for the last two months. The issue is that she is suffering from acute pain in the lower back. She cannot move at all. She is taking Calpol 650 mg is drive away the pain but it is coming back again. Also she cannot move her hands easily as the pain is there also. What to do?

Hello XXXX,
Welcome back. I remember her medical history well. In my opinion if she is suffering from chronic back ache, she should be evaluated for any destruction of vertebrae of her lumbo-sacral spine by tuberculosis if back, that is Pott's spine. In a lot of cases of Pott's spine, there is destruction of the vertebrae causing the back pain.
Another possibility is joint pains due to the side effect of anti-tubercular drugs.
So, i would advise for her MRI Lumbo-sacral spine, which will rule out any evidence of disc prolapse. Also, i would advice to get her serum uric acid repeated.
For the purpose of pain, she can take a combination of Tramadol and Acetaminophen. That will give her relief from the pain, but we need to evaluate the cause behind her pain.

Hi Doctor,

She siddenly develped the backpain last Thursday after she got up from sleep in the morning. Before also she developed this isue in her hands which slowly became normal after a few days.

The issue is that she developed it suddenly. I checked her back. Only at one part she is having the pain and it is spreading only when she is walking. Last Wednesday she went to the market to buy vegetables. Suddenly after coming back she said that she is having pain and it escalated.

Then it might be muscular in nature.

Detailed Answer:
Hello, In such case she might have developed Acute Muscular Back Pain. For muscular pain, i advise to take a combination of pain killer and muscle relaxant containing Diclofenac and Thiocholchiside. She should do hot fomentation of the affected area and apply Diclofenac ointment locally and get physiotherapy done particularly Ultrasonic heat massage.
If she doesn't get relief in next 5 days then she would require to get MRI Lumbo-Sacral Spine done.

Hi Doctor,

She is having levofloxacin. Is this the side effect of the drug? She has pain on her limbs but much less. She consulted an orthopedic about one and half a months back and he told her that levofloxacin leads to pain in the limbs.

Unlikely.

Detailed Answer:
Hello, Since she is taking Levofloxacin since long, it's very unlikely to be due to that. Levofloxacin has no prominent effect like pain in limbs. It can cause problem with limbs in neonates and baby of a pregnant female.
I would advice you to supplement her with Calcium and Vitamin D.
I don't think any particular intervention is required.
