There might be something in your bathroom which you might be inhaling due to which you are suffering from Allergic reaction in your nose(Allergic Rhinitis) followed by inflammation in your ear.From the pictures you posted of the skin presentation on your hands,it seems that you might be having contact with something ,that you are suffering from allergy on skin(Allergic Vasculitis-inflammation and damage of small blood vessels over skin on coming in contact with an allergic substance)
For the treatment,if possible you should try to avoid coming in contact with the things which trigger your symptoms-if possible try to avoid going in that room/bathroom or when needed use a face mask to prevent inhalation of allergen and wear gloves to prevent allergen coming in contact with your skin.
It is advisable to do few blood test like Complete blood count with Erythrocyte sedimentation rate-it will help to find if you have any infection,platelet count if reduced may suggest blood coagulation disorder,Eosinophils if high may suggest Allergy and Blood coagulation test to rule out blood coagulation disorder.
For the treatment of Allergic reaction in the nose followed by inflammation in ear leading to ear pain,you may continue with the Fluticasone spray as advised by your doctor.You may also get relief by taking Oral Decongestant(like Phenylephrine) if you are not suffering from Hypertension,Nasal decongestant Drops (like containing Xylometazoline),Antihistaminic(like Cetrizine),Analgesic(like Ibuprofen) along with antacid(like Rantac).Also do steam inhalation.Avoid cold food and dust.Avoid self-cleaning of ear.Drink lots of fluids(warm).
For the treatment of probable Allergy reaction in your skin(Allergic Vasculitis),You may need Anti-inflammatory medicine like Ibuprofen, or corticosteroids to reduce inflammation of the blood vessels and Antihistaminic (like Cetrizine) which will help against the allergic reaction.Also,don't scratch your skin if you are feeling itchy.
