Ian 70 yes.with diabetes and had bypass surgery.now since 3months suffering from fissure without any relief from medicine.pls advise

I would like to know if you are talking about anal fissures.
If you are talking about some other part with fissures please let me know.
Anal fissures largely need treatment with surgery.
I will offer to you the medical management for the same.
If that does not help you will need to seek help locally with a surgeon.
The following are medical options.
1. high fiber diet.
2. plenty of fluids, if no restrictions from the cardiologist.
3. stool softeners like isapghulla husk or lactulose solution.
4. a short course of antibiotic metronidazole.
5. avoid straining at stools.
These are the medical options.
