Suggest Treatment For Anorgasmia While On Antidepressents

default
Posted on Mon, 30 Mar 2015
Question: I have tried ssri's and other types of antidepressants. None of them have alleviated the symptoms except for 10 mg dexedrine, 3x a day. The problem: I have a strong libido, but can not have an orgasm like I used to before taking the medication. The frustration is causing anxiety and a salivary cortisol level that is "off the charts" So far, hormonal creams have not helped. What type and dose of creams would you suggest?
Or, is there anything else I might try? I'll be continuing the dexedrine. I also take 2 mg of klonopin at night. Thank you very much for your advice!

Brief Answer:
estrogen therapy or 5-PDE inhibitor group of medicines can be tried

Detailed Answer:
Hello XXXXXXX

Thanks for writing to us.

Your problem is not able to get climax though you have sufficient libido.

This condition is called anorgasmia.

After menopause this type of problem is likely related to hormonal problem.

You are already on testosterone therapy.Adding estrogen may help to achieve orgasm.
Two types of route of estrogen therapy:
Systemic estrogen therapy — by pill, patch or gel. It can have a positive effect on brain function and mood factors that affect sexual response.
Local estrogen therapy — in the form of a vaginal cream or a slow-releasing suppository or ring that you place in your vagina. It can increase blood flow to the vagina and help improve sexual arousal.

Other option is to use 5-PDE inhibitor group of medicines like sildenafil or tadalfil. One of them should be taken before 2 hours of sex.

Discuss above options with your treating doctor and start one of them.

To help you further I need to know some more information.

-Since when are you suffering from this problem?

-SSRI was given for which symptoms? Which SSRI?For how much duration?
SSRI itself can cause sexual dysfunction. Dexderine has no sexual side effect. It may rather help to improve libido.

-How is relationship with your partner in regard to intimacy? Have you ever gone through couple counseling for sex?Have you tried sensate focus or masturbation?

-Do you have any physical problem and are you taking any medicine apart from mentioned here?

Hope I have answered your query, I will be happy to help further.

Regards,
Dr.Chintan Solanki.
