Question: I am anxious before getting into bed, sleep for a couple of hours, wake up with trembling and palpitations, and very anxious, not so much about the trembling etc but afraid of EVERYHING, these last about 1/2 hour, trouble getting back to sleep, then the whole thing is repeated another 2 or 3 times. very anxious during most of the day with aching stomach and chest. I have tried hypnosis, counseling, CBT,
all to no avail. I have become very frightened of most activities This has really badly upset my confidence, I think of suicide very often.

Brief Answer:
Medicine can help you, consult psychiatrist,do not act on suicidal thoughts

Detailed Answer:
Hello ,

Thanks for writing to us.

I can understand your concern.

Your symptoms are suggestive of insomnia, anxiety and depression. They all are interrelated and one can occur in other problem. For proper diagnosis detail history and mental status examination are to be done by a psychiatrist. Physical causes should be ruled out with physical examination and necessary investigations.
Suicidal thoughts suggest likely possibility of depression. I request you to do not act on such thoughts. Your problem is treatable.

Whatever the diagnosis may be, benzodiazepine groups of drugs like lorazepam or clonazepam can help in such conditions very effectively. They do not have significant side effects when used under medical supervision. Diazepam is from same group. Yes these drugs are addictive when taken without medical supervision. But when used under medical guidance they are very helpful and can be stopped gradually.
Psychiatrists deal with such problems and they are used to with such type of prescription and they do not hesitate to use these drugs. If I were your treating doctor I would like to give lorazepam considering your age.

When problem is severe counselling,CBT or hypnosis do not work.

Consultation of a psychiatrist will help to diagnose your problem clearly and you can have necessary prescription.

Hope I have answered your query, I will be happy to help further.

Regards,
Dr.Chintan Solanki.

Thank you for your answer, do I have to ask my doctor to arrange for me to visit a psychiatrist, and would the psychiatrist arrange for me to be physically checked out, presumably to make sure that no chemical imbalance exists, I do reside in UK.
