Suggest Treatment For Anxiety Related Premature Atrial Contraction
Question: Hi I have PAC as per my holter results. I've had all sorts of other tests by my cardiologist including stress test, echo, ECG. My stress test showed completely normal, echo showed structurally normal heart. My holter showed PIC'S I believe 925 of them. I'm worried about this and I do suffer from anxiety quite a bit my cardiologist says it is anxiety related. Should I still be worried? It's all I think about
Brief Answer:
Nothing serious
Detailed Answer:
Dear Sir,
You shouldn't worry. PACs are very common in young and healthy adults and especially if there is anxiety.
They are considered bening. Usually there is no need for treatment, but if you are symptomatic, you can discuss with your cardiologist the possibility of prescription of a low dose beta blocker.
Your cardiologist is right, most likely they are related to anxiety. So you need to see a psichiatrist and discuss the treatment strategy.
Anyway, to be in the safe side, I would also advise you to undergo thyroid tests and serum electrolytes.
Hope I could help you
Wish you good health
Please, let me know if you have further questions
Regards,

Hi Doctor that's great to hear. I forgot to mention the 925 PAC were over a 48 hour monitor. It also said 1 single ventricular beat. Don't know what that means. I noticed sometimes when I eat I'll get bloated and my heart will start to race. The monitor also said my heart rate is around 56 26% of the time and in tachycardia 12 % of the time. All in sinus rythm. No prolonged pauses and the slowest heart rate was 50 during deep sleep at 5 AM. What does all this mean?
Also I feel them usually when I wake up in the morning then they are not that noticeable throughout the day. Is this normal to have them during sleep

Brief Answer:
Everything is within the physiological limits
Detailed Answer:
Dear Sir,
It is even better that they are for 48 hours. It means around 460 PAC a day which is OK. Your follow-up query was quite detailed, I went through it and everything is OK and within physiological limits.
A single ventricular contraction can be in any person.
Heart rate usually slows down while we are asleep. The slowest is near the morning, so 50 bpm on 5 AM is quite normal finding.
After eating, it is reflex when heart starts race. When we eat much, our stomach gets bigger, it presses diaphragm and we can have tachycardia or some arrhythmias. So, try to eat with small portions, avoid taking liquids with meal.
That small amount of PAC you can have during the hole day, also at night. They can be more in the morning when your organism wakes up and usually the amount of adrenalin increases. This is physiological reaction and there is nothing to worry.
Wishing you good health
