Question: Is surgery indicated if the patient shows signs of dense arteriosclerotic calcifications involving the common iliac arteries and to a lesser extent the internal and external iliac arteries bilaterally?

Surgery (or rather minima invasive procedure done from inside of the blood vessels) is indicated if you have any symptoms associated with legs ischemia. This symptoms are calf pain while walking, cold and pale feet, skin atrophy, legs muscles degeneration.
Most of the patients at your age will have some arterosclerotic calcifications. The point is how narrow is the blood vessel and if it is wide enough to allow proper blood circulation.
Please consult vascular surgeon with this issue.
