Detailed Answer:
It looks that situation is one of the complicated one.
In your case prime importance should be given to control & keep diabetes in limit.
As there is metallic implant in body & there is presence of pus. This is making the situation one of the complicated one.If your doctor has checked for effectivness of antibiotic on infective organism & tried antibiotic for at least 3 weeks then you should ask for removal of metallic implant to give relief from infection as well as pain.
In such severe pain i used to give tramadol as an analgesic.YOu may ask for this from your doctor.
Hope this will help you.
With best wishes.

she is not in a position to take any surgery or intravenous fluids since her lungs are filled with lot of fluids and her heart is not functioning to its capacity to undergo surgery for removal of the metallic plate so a procedure to get rid of pain without surgery ,intravenous fluids .your appropriate advise in this case will be welcomed
Brief Answer:
It looks that your main concern is her pain.
Detailed Answer:
Yes,If your main concern is her pain then you should search for a good anestesiologist & critical care specialist who may administer thoracic epidural analgesia to her.Your doctor may look for feasibility of this procedure in her case.If she is fit for that this is very good procedure to give her relief from pain.
With best wishes.
