Nose bleed on straining is present mostly due to hypertension or some nasal pathology.
Renal disease can lead to hypertension. My advice is you should definitely visit ER and get yourself examined. If there is still a bleed, get yourself examined by an ENT SPECIALIST as a nasal packing may be required.
if you are a hypertension patient, continue with the antihypertensives.
