I have gone through your query and I have understood your concern.
After going through your history I can make out that you probably suffer from a moderate to severe grade of inflammatory acne.
I would like to know a few more details from you so that I am able to guide you better.
-do you have any irregularity of menstural cycle? Do you have any problem with coarse facial hair?
I noticed that you are taking ATT (Antituberculosis treatment) of which Isoniazid is an important component. Isoniazid causes acne as a side effect. This might be one reason for your acne worsening in recent times. However, isoniazid has to be continued as part of tuberculosis treatment.
I would suggest you to use a topical gel, Clindoxyl gel, a combination of clindamycin and benzoyl peroxide (1% clindamycin+5% benzoyl peroxide). You may use it twice daily for active acne lesions.
In addition I would also suggest an oral antibiotic (cephalexin) i.e Cap. PhexinBD (750mg), twice daily for 2 weeks.
Use a gentle cleanser for face wash e.g cetaphil cleansing lotion, twice daily

But my point is I am facing acne problem severely because of tb treatment.But I am suffering from acne since 12-15 years when will I be able to get rid of this problem. One thing I would like to add my menstural cycle was irregular when it started first time and it was irregular 8-10 months then I got treatment of the same.

I agree. I consider Isoniazid to be only an exacerbating factor not a primary cause.
Melacare cream can cause acne and increase facial hair because it contains a potent steroid i.e mometasone furoate.
I would like you to upload a few Images of the acne affected area, for my review. You can upload Images in "Reports Section".
Can you also enumerate what all kind of treatments for acne you were on previously? So that I can add to Or modify what I have already suggested and how effective were those previous treatments?

Thank you. I have reviewed the Images and prescription.
You should continue with neutin capsules. It contains isotretinoin and is one of the best oral antiacne remedy. It should be continued for at least 6-8 months.
Apart from that I suggest you to use topical microbenz % gel during daytime and topical Deriva-CMS gel for night application.
In addition, I suggest you to use Cap Phexin BD 750 mg twice daily for 2 weeks.
Facial hair would respond to Laser. You may go for a few sittings of Laser hair reduction.
